New Client Form 2026


By submitting the above New Client Form, you agree to the following Store Policies :

Appointments & Scheduling

  • Please be on time or up to 15 minutes early to your scheduled appointment.
  • Cancel/ Reschedule your appointment at least 24 hrs prior to your visit. Late arrivals & No-Shows may incur a fee equal to 50% of the scheduled service. Late arrivals (we allow up to 15 minutes) may result in a shortened service, rescheduling, or late fee. 

Picking Up Your Pet

  • Pets must be picked up within 1 hour of our Text/Call when the service is complete. We are limited on space and provide an Express service to our clients. Late pick-ups will incur a $15 charge for every hour they are in our care after said call/ text.
  • *In the event of a Pet Parent Emergency Pets not picked up by closing time may be boarded overnight at the owner's expense and the Pet Parent assumes all risks. A $25 Care Fee is charged plus any additional expenses.

Health & Vaccinations

  • We reserve the right to refuse service if your pet appears ill or poses a health/ behavioral risk to others
  • Pets must have a current Rabies Vaccine (within 6 months) or a Titer Test document signed by your Veterinarian. 
  • Please inform us of any health issues, allergies, or behavioral concerns regarding your pet prior to your visit. 
  • Fleas- If your pet has fleas , a mandatory Flea Service will be added to your base price ($25 upcharge)

Matting & Special Grooming Needs

  • Severe Matting may require a short clip for the pet's comfort & safety. This can cause skin irritation or changes in coat appearance- prevention through regular grooming is best.
  • De-Matting Fees apply to moderate to severe cases. We will not cause undue stress to your pet through excessive dematting.
  • Senior Pets or those with health issues will be groomed gently with a priority for cleanliness & comfort. 

Agressive or Difficult Pets

  • Please disclose ANY history or biting, aggression, or anxiety. Muzzles and other safety restraints will be used if needed.
  • We reserve the right to refuse or stop service for safety reasons. Full service charge still applies if the groom is interupted.

Payment & Photos

  • Payment is due at pickup. We accept Cash only as of 1/1/26 unless otherwise discussed with the salon owners.
  • We may take photos of your pet for records or promotion (with the owner's permission; Opt-Out available). Pet Parent's personal information will never be made public.

Safety & Liability

  • Grooming involves risk (e.g., minor nicks, stress reactions) We take every precaution, but accidents can occur
  • In emergengies, we will seek the nearest Veterinary Care Facility and contact you- owner is responsible for all costs.
  • Owners are liable for any damage or injury caused by their pet to staff, other pets, or salon property.
  • By signing, you release Lap Of Luxury Canine Salon, its staff, and Owners from liability for injuries, illness, or issues arising from know/unknown conditions, except Gross Negligence.
  • We reserve the right to refuse service at our discretion
  • Policies may change without notice
